The bit about the 'Harrow Formation' makes me think we could probably have an entire article dedicated to "Things nobody ever wrote down because they seemed self-evident at the time, and are now lost". I recall a few other examples being mentioned.

Crossbowmen frequently worked with an assistant who carried a shield, a mantlet, to protect them both from incoming fire.

It reminds me of a historian (forgot who) who compared medieval knights to modern tanks, in that they were a crew-operated affair rather than a single individual.
